In times of illness, people often ask “let me know what I can do to help”, but in crisis moments those things can be hard to define, especially if asking for help isn’t something you’re used to.
This Constellation Chart and guide were created to help organize your own Care Constellation - allowing you to anticipate your own needs (in illness or as a caregiver), and pre-arrange asks so that when the times comes your constellation will know how to show up for you.
The idea for the chart was inspired by Pod Mapping which was developed by the Bay Area Transformative Justice Coalition, and also my own experience helping people delegate care asks and coordinate support at end of life. I’ve seen this type of planning offer ease, connection, and better overall coordination of care in serious illness and at end of life.
